LifeCare Solutions (LCS) is an Accredited Home Health Agency dedicated to providing the highest Quality Homecare to our patients. LCS is opening a Home Health branch in Palm Beach County. We are
currently looking for the following Full time, Part Time, and Per Diem positions:

Speech Therapist
Registered Nurse
- Provide skilled nursing care to patients including: Assessments and reassessments
- Patient and caregiver teaching
- Development and revision of the Plan of Care and Plan of Treatment
- Implementation of physician orders
- Psychosocial support.
- Pre-discharge patient assessments when appropriate
- Complete all required patient care documentation and reports in accordance with company P&P’s
Licensed Practical Nurse
- Provides skilled nursing care to patients in the home per physician order and patient care plan
- Accept assignments and supervision of RN for follow up home patient visits
- Evaluate and report findings to supervising RN
- Assist RN to develop Plan of Treatment on an ongoing basis with physician supervision
- Document clearly and accurately all nursing actions on appropriate forms
- Submit all paperwork in the specified time frame in accordance with company P&P’s
Certified Nursing Assistant:
- Administer or assist patients with personal care including bath or shower, skin, hair, mouth, and nail care
- Assist patient with elimination using bathroom, commode, bedpan, or urinal
- Monitor and record vital signs
- Assist patient with dressing or undressing and positioning for comfort
- Assist patient with prescribed exercise after instruction by the Registered Nurse, Physical Therapist, or Occupational Therapist
- Prepare meals and assist patients with eating
- Assist with household services that are essential to patient’s home health care
- Document accurately and concisely all care and observation
- Report pertinent observations and changes in the patient’s physical and psychosocial status to the RN, Therapist or Manager
Physical Therapist
- Assess the patient to determine current level of function and type of physical therapy services required
- Perform active and passive exercises: muscle reeducation, gait and functional activities of daily lifting, including transfer activities ensuring pain relief and function restoration in
condition to physician and health care team
- Evaluate the home environment and makes appropriate recommendations
- Instruct patient in the care and use of assistive devices, e.g. wheelchairs, braces, crutches, canes, as well as prosthetic and orthotic devices
Physical Therapy Assistant:
- Provide care according to physician’s orders, assessment data, and established standards and guidelines.
- Perform direct physical therapy services in accordance with accepted standards of practice and certified by the patient’s physician
- Instruct patient and/or their family in the appropriate and safe use of prosthetic, orthotic, and assistive devices (canes, walkers, wheelchairs, etc.)
- Instruct the patient and/or caregiver in the home therapy program and clarify Counsel and instruct the patient and family to meet therapy and related patient needs
Occupational Therapist
- Assess the patient to determine current level of function and occupational therapy services needed
- Apply diagnostic and prognostic procedures, including cognitive testing, to evaluate the patient’s level of function in activities of daily living, including eating, dressing,
food preparation, and other household activities
- Guide the patient in the use of therapeutic, creative, and self-help activities for the purpose of improving function
- Observe, record, and report the patient’s reaction to treatment and any changes in condition to the physician and health care team
- Evaluate the home environment and make appropriate recommendations
Certified Occupational Therapist Assistant (COTA)
- Provide care according to physician’s orders, assessment data, and established standards and guidelines
- Document progress toward identified objectives
- Observe, record, and report the patient’s reaction to treatment and any changes in the patient’s condition to the physician and health care team
- Instruct other health care personnel, caregivers, and/or family members in methods of assisting the patient to improve and correct functional and cognitive disabilities
Speech Therapist
- Provide care according to physician’s orders, assessment data, and established standards and guidelines
- Assess the patient to determine type of speech, hearing, or language disorder and the appropriate corrective therapy
- Provide rehabilitative services for speech, hearing, and language disorders
- Develop and implement new program/techniques to enhance speech-language pathology services
- Graduate form an accredited school of speech pathology, registered by the State Board and meets the requirements for a Certificate of Clinical
- Competence in speech pathology or audiology
