Registered Nurses Needed - All Areas of Town - All Shifts
Registered Nurse Case Manger - State Licensed Homecare
(PAY UP TO $45 / hr)
- RN Accepts client cases based on nursing skills and geographic location.
- Admits clients to Dependable Nurses, Inc.- Homecare Division.
- Instructs client /caregiver in patient rights and responsibilities.
- Advance directives and financial responsibilities.
- Completes assessment of home environment and safety.
- Completes physical assessment.
- Initiates plan of care for skilled and supportive services.
- Manages client's case.
- Supervision of supportive staff is every 60 days or more as indicated.
- Supervision of housekeeping staff only is 180 days or more as indicated.
- Reports to nurse covering caseload in absence.
Registered Nurse Case Manger - Medicare Certified Home Health
(PAY UP TO $65 / hr)
- RN's Complete an initial assessment of patient and family to determine home care needs.
- Provides a complete physical assesmentand history of current and previous illness.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family
in the planning process.
- Provides health care instructions to the patient as appropriate per assessment and plan of care.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
- Acts as Case Manager when assigned by Clinical Supervisor and rn assumes responsibility to coordinate patient care for assigned caseload.
- Full Time Registered Nurses required to perform 27 visits per week to maintain full time status.
- Participates in on-call duties as defined by the on-call policy
- Attends In-service Every Month
- Attends and Participates in Patient Care Conference 2 times per month.
